Enable Visual Assist in second instance of Visual Studio 2008

Visual Studio 2008 and Visual Studio 2005, using Windows 7

By design of Microsoft, instances of the IDE launched with Shift+Left-Click are loaded without extensions, including Visual Assist. (Left-Click alone brings focus to a running IDE.) 

Launch a second instance of the IDE with Visual Assist, and other add-ins, using Shift+Middle Click.
